Established in 1820, the United States Pharmacopeia is an independent and nonprofit science-based public health organization. It is also a manufacturer of various health care products and provides its products throughout the United States. It operates two programs for patients who take medications, such as the medication errors reporting and MEDMARX programs. Its medication errors reporting program allows health care professionals to directly report medication errors to the organization. Its MEDMARX program is an Internet-based medication error and adverse drug reaction reporting program. It additionally provides information that supports the health care practitioners and pharmacists in the research and development of patient safety programs. It is located in Rockville, Md.
