R&T Elder Care Services, also known as Retirement & Transition Elder Care Services, offers health care management services to senior citizens. The facility provides information about retirement communities, nursing homes and other living alternatives. It offers discharge planning, monitoring and telephone reassurance and companionship services. R&T Elder Care Services provides assistance with the activities of daily living, such as housekeeping and meal preparation.
