Oak Forest is one of the few skilled nursing communities in the state that features a dedicated Ventilator-Dependent Care Unit with 18 semi-private beds, a high staff to patient ratio and a 24/7 respiratory therapist. When you arrive at Oak Forest, you are greeted by a clinical team who develops a person-centered care plan designed to help you regain skills and decrease the chance for readmission back to the hospital. You also have access to medical services that address secondary conditions for a complete treatment plan.
