The Delaware Center for Homeless Veterans, Inc. (DCHV) is a 501(c) (3) certified nonprofit and the only Veterans-specific nonprofit in Delaware utilizing the Housing First Model. DCHV was honored and named Veterans Reentry Model Program by the Department of Veterans Affairs. We are experts in the areas of VA Benefits applications and appeals process and securing benefits for needy veterans. Expanding on the successes of our teams' housing, social services, drug and alcohol assessment and treatment counseling, and developing supportive services and programming for the homeless and low-income veterans in Delaware, we are focusing our efforts to combat the issues and obstacles facing homeless veterans in Delaware. Along with the Wilmington VA Medical Center, other VISN 4 facilities and staff, the Homeless Planning Council of Delaware, and we assist in affecting a change in the number of homeless veterans, through affordable housing, community development innovation and enhanced service plans, including discharge plans, and substance abuse treatment programs. Our efforts directly affect the homeless veterans we serve and all Delaware residents, many of whom are directly affected by homelessness and its risks. Starting with the Department of VA Emergency Shelter and Contract Beds Programs, leading up to the Department of Housing and Urban Development (HUD) and the Department of Veterans Affairs HUD-VASH Program (permanent supportive housing program) we have increased our coordination and collaboration with our local VA Homeless Care Veteran Outreach Coordinator, VA VISN 4 Reintegration Coordinator and local stakeholders private and public to develop programs and services for a more seamless delivery of wrap around services to homeless veterans.
