After years of success in the senior residential healthcare community focused on patient- and resident-centered care, we understood the critical importance of a more holistic approach that recognized the importance of attitude, positivity and self-confidence in the healing process. Our Home Health plans of care begin with a personal and private consultation, during which we will determine your individual level of care and coordinate the required services. Your plan may draw from any or all of Love My Nurse's professional disciplines, depending on your needs, and will be developed in concert with your family and other health care providers to encourage their active involvement in your ongoing care. Once the home health care plan is in place, your liaison will also ensure that your needs are being met and schedule additional services on an as-needed basis or at your request.
