CoRNerstone Health is a healthcare maintenance organization founded by registered nurses. We offer Corporate membership and healthcare solutions for local corporations looking to improve employee health and benefit engagement and lower overall health-related costs. Services will include well-being and ROI assessments, on-site education, overall care management to include medication management as well as guidance on medical insurance billing or coverage issues, focused “challenge area” health campaigns, nurse hotline, access to personalized healthcare navigation with a registered nurse (each member has a team of three nurses that they will be assigned to so that they are always talking to a caregiver that is familiar with their health), follow-up care engagement and crisis care management for unexpected traumas, unexpected diagnosis, emergent care situations or other crisis-like situations. The nurse assigned to the crisis care will take over as one of the three nurses assigned to the patient for any type of continued membership. We know that seamless care is key to success. Provider and private memberships will offer an extra layer of support and act as a liaison between the patients, families, caregivers, insurance carriers and the providers to optimize patient outcomes. Membership will offer one personal registered nurse team at the center of all healthcare related needs, providing a bridge from one service and/or specialty area to the others for a holistic focus. This service stands to significantly decrease hospitalizations, readmissions, medication misuse, medical non-compliance and poor outcomes. Membership can improve the effectiveness of patient-provider communication, patient engagement as well as patient satisfaction and overall healthcare success. Additional services to empower patients to shop for services based on actual price and quality of care are currently in development for all membership levels.
