Founded in 1992, Forte Residential is a nonprofit organization that assists and supports individuals with various physical and developmental disabilities. The organization provides waiver services in a number of counties in Indiana, such as Allen, Cass, Dekalb, Elkhart, Fulton, Huntington, Kosciusko, Lagrange, LaPorte, Marshall, Miami, Noble, Porter, Saint Joseph, Starke, Wabash and Whitley. It provides in-home support for individuals receiving services through the Medicaid Home and the Community Based Waiver System. Forte Residential is certified to provide services through several Medicaid waivers, including developmental disabilities and support services, autism and the aged and disabled. Its services include residential habilitation and support, family and caregiver training, respite services, attendant care and transportation assistance. Forte Residential is located in Syracuse, Ind.
