Operational for more than 25 years, Project Transition is a community that offers accommodations and medical assistance to people with psychiatric problems. It provides various medical programs-related to personality disorders, mental health, and substance abuse and traditional diagnoses. The community offers treatment, skill development programs, wellness training and supervised independent living. Its amenities include swimming pools, health clubs and shopping centers. The community conducts various family activities, including welcoming orientation, educational seminars, and multifamily support and progress meetings. It offers psychiatric hospitalization, medication, and drug and alcohol rehabilitation programs. Project Transition manages a staff of professionals, including psychiatrists, psychologists, psychiatric rehabilitation counselors, program managers and wellness coordinators.
