- Primary Care1. Treatment. We will use your medical information to treat you. For example, we may disclose your medical information to other doctors, nurses, technicians, medical students, or other members of our staff who are involved in taking care of you or to other care professionals for additional treatment or follow up care such as home health services. We also may disclose your medical information to people outside our medical practice who may be involved in your care such as your primary care physician or family members.
- Emergency CareEmergencies do occur, and everyone in our office will do their best to respond to your problem quickly. In the event of a severe situation, or one in which you are in doubt, go immediately to the Emergency Room and ask them to contact one of our physicians. If the situation is not severe, but one where you wish to contact the doctor, please telephone the office and the doctor will be in touch with you either directly or through his staff, giving you instructions on how to best handle your problem.

- Orthotics and Prosthetic Therapy20. Organized Health Care Arrangement. Oklahoma Spine and Brain Institute (“OSBI”), its physicians, and other health care providers provide care in a clinically integrated care setting and constitute an organized health care arrangement under HIPAA. As of the date of this Notice, the “other health care providers” who provide services to the patients of OSBI within the clinic include First Step Orthotics and Prosthetics, LKM Enterprises, and RX Medical, and their employees. The organized health care arrangement involves participation of legally separate entities in which no entity will be responsible for the medical judgment or patient care provided by the other entities in the arrangement. All entities, however, have agreed to abide by this Notice of Privacy Practices (NPP) while working in the clinic setting known as OSBI. If you see a representative of one these named providers outside OSBI, you will receive another NPP from each of them upon your first encounter in their offices, which may be different from this NPP and which will govern the protected health information maintained by that provider. The physicians and other health care providers at OSBI will be able to access and use your Protected Health Information to carry out treatment, payment or health care operations for the clinic.
