Established in 1984, Hospice of Lake & Sumter is a private, nonprofit, community-based health care organization providing various basic and palliative care services to the people of Lake and Sumter counties in Florida. The center provides regular nursing visits, spiritual counseling, volunteers, medications, medical equipment, supplies and therapies. Additionally, it provides transportation, social services, hospice, physician, family, personal, patient and continuous care services. The center also provides emotional and bereavement support, and acute, respite, long-term and crisis care services. It also provides special programs, such as Pathways Program, Children s Program Life Enrichment and Tuck-In Program. The facilities of the center include various hospice field offices, the Villages Hospice House, Tavares Hospice House and Hospice Center. Hospice of Lake & Sumter is located in Tavares, Fla.
