Nazareth Health and Rehabilitation Center is a provider of health care services to the residents of Stoughton and Madison area. Based in Stoughton, Wis., the center offers short-term as well as long-term stay after a hospital recovery. It provides rehabilitative programs, including physical therapy, speech therapy, respiratory therapy and occupational therapy. The nursing home offers round-the-clock nursing care; hospice/palliative care, dementia care and respite care services. Additionally, it offers a range of treatment for residents requiring IV therapy, antibiotics, dialysis and nasogastric tubes. Nazareth Health and Rehabilitation Center is licensed by the State of Wisconsin and managed by Health Dimensions Group.
