Our Story Avalon Care Center at Northpointe offers both short-term and long-term placement for patients recovering from either an accident or injury requiring skilled nursing and/or skilled therapy care. We offer a 20 bed sub-acute unit with a committed team of nursing and physical, occupational and speech therapy professionals. We have a dedicated Medical Director and supporting medical staff that provides medical supervision and assessments at bedside. This dedication provides the patient with a continued focus on recovery and well-being while at Avalon Care Center. We provide the care and services necessary to help the patient make a successful transition home.

My mother went there for rehabilitation for a hip replacement. She arrived on a Friday afternoon and was not even evaluated until Monday afternoon when I asked what their plan of rehabilitation was. She had been either sitting in a wheel chair or put into bed up until that time.. The physical therapist was rude to the point that I needed to talk with the DNS. By the time she had any time of physical therapy it had been 72 hours since she had put any type of weight on her leg. The previous night she had asked for a pain pill at 8:00 p;m; and finally recieved it at 10:00p.m.
