  To request a copy of immunization records from Texas Immunization Registry (ImmTrac2), please complete and submit (by fax or mail) an Authorization to Release ImmTrac2 History Form. Please allow 10-14 business days for delivery of your immunization records from the time of submitting your ImmTrac2 request.
