General Info

In Saginaw, Michigan, and as part of Ascension Michigan, Ascension St. Mary's Hospital Center of HOPE community health workers currently provide preventive screenings for obesity, diabetes and hypertension. We offer health education, including the proper use of insulin and other diabetic supplies, follow-up care, and proper diet and nutrition. The Center of HOPE help provide access to primary care, especially to those who are most vulnerable.
