- Primary CareWHAT TO EXPECT ON YOUR FIRST VISIT In order to have a hearing test, vestibular testing, or hearing aid evaluation you ears need to be cleaned and free of wax. If you believe you have an ear infection or wax (cerumen) in your ears, please call your Primary Care Physician or Dr. Chahfe’s office to have your ears cleaned and checked prior to your visit. Please let the office know if you have a prior hearing test or if you have ever worn hearing aids before. Bring all insurance cards and a list of your medications to your appointment. If this if the first time you are being see in the Audiology office you will be asked to fill out a patient case history form which can be found on this page, can be mailed or faxed to you, or you can pick up a copy at the office.

- MRIWHAT TO EXPECT ON YOUR FIRST VISIT You will receive a call from our office a couple of days prior to your appointment. A nurse will go over your medical, surgical, allergy and family history with you. She will ask you for your current medication list and any current tests you may have had such as x-rays, CT scans, hearing tests or MRI’s. This information will be entered in the computer before your visit and briefly reviewed with you on the day of your visit. If we were unable to reach you prior to the appointment, you will be asked to complete a form on arrival. Please be sure to bring a list of your medications and the reasons you are taking them.
