Home Health Care, a service unit of Unity Health System, is an in-home health care service provider. It offers individualized care planning and medical and nonmedical care services for individuals who suffer from prolonged illnesses and injuries. The unit provides skilled nursing, risk factor assessment, long-term clinical care and family educational services. It also offers referral and medical coordination services to provide access community health care resources. Home Health Care also offers assistance with Medicaid applications and placement for nursing homes, adult living centers, assisted living communities and senior housing organizations. It works with patients primary physicians to provide medical treatment and diagnostic services. Home Health Care maintains a presence in Rochester, N.Y.
