  Kathy has been with Southern Rehab since 2015. Kathy has over 25 years of experience in Case Management, including home care, hospital discharge planning, utilization review, consulting and workers’ compensation. Prior to case management, Kathy worked in a hospital setting in the Med Surg, Emergency Room and subacute unit.

  Leslie joins the Southern Rehab Case Management team with over 10 years of nursing expertise. Prior to coming onboard with the company, Leslie served Veteran patients on the Endoscopy Unit. Leslie has expertise in dialysis, pulmonary, ER, ICU and endoscopy.

  Her strengths as a Medical Case Manager are building strong relationships with patients and adjusters and communicating effectively to achieve the best outcomes.  Susan maintains clinical knowledge in the following nursing areas and WC cases: CAT, multiple injuries, TBI, ortho, neurology, burns, and amputees.

  She has been with the Southern Rehab Case Management team since April 2024.  She joins the team with 19 years of clinical nursing experience. Prior to coming onboard with the company, Gita worked as a Clinical Nurse Reviewer performing quality reviews and assessments. She also served in a leadership role at a hospice facility. Gita maintains clinical expertise in the following areas: Progressive Care, MedSurg (including ortho, general surgery, and gyn surgery) hospice, and home health.

  With over 29 years of nursing experience, 6 years of workers compensation and field case management experience, Anissa maintains strong relationships with adjusters while coordinating care for her clients.  Case management expertise in the following areas: CAT, orthopedics, pain management, trauma, and coordinating IMEs.

  She joins the team with over 24 years of clinical nursing experience and 10 years of home health case management expertise.  Prior to coming onboard with the company, Patricia worked as a nursing supervisor and as a school nurse.  Areas of clinical focus include Med/Surg, telemetry, and acute care, diabetic instruction, and physical therapy.  Patricia has experience in serving pediatric, adult, and geriatric populations.
