  I hereby declare that the information provided by me in this application for employment is true, correct and complete to the best of my knowledge. I understand that if employed, any misstatement, false statements or omission of fact on this application shall result in rejection of this application or termination of employment. I understand that this application is valid for thirty days. I must reapply to be considered after this period. I understand that all job offers made by an authorized representative of IPSA Security Services are contingent upon passing a drug screen, presenting documents to conform with the Immigration Reform and Control Act and form I-9, and background investigation. In consideration of my employment, I agree to conform to the rules and regulations of IPSA Security Services and that my employment and compensation can be terminated with or without cause, and with or without notice at any time, at the option of the company or myself. I understand that only the Chief Executive Officer of the Company has the authority to enter into any employment agreement for specified period of time and that such agreement must be in writing. I also agree that if I am hired, as consideration of my employment I will be bound by and accept as a condition of my employment, the terms of the IPSA Dispute Resolution Plan Rules and Summary Plan Description (IPSA Dispute Resolution Program).
