Integrated Care Program Our new PowerCore for Life Program provides our patients with a comprehensive integrated approach for their care that is designed to enhance their quality of life, restore their function and reduce their pain. Our new program name references our patient's spine through the word "Core", functional restoration through the word "Power" and lastly enhancing their quality of life through the word "Life". Prairie Spine & Pain Institute, S.C. will be the exclusive provider for the PowerCore for Life Program throughout Illinois, as we have developed and filed for trademark protection for our unique new clinical model. THE INTEGRATED PRACTICE UNIT: The "Integrated Practice Unit" or "IPU" is the cornerstone of many initiatives to improve healthcare. Prairie Spine & Pain Institute has elected to build our new PowerCore for Life program to embrace the goals of enhancing the delivery of healthcare through single site acute or chronic disease management. These goals are outlined in the book "Redefining Healthcare" authored by Professor Michael Porter of Harvard Business School. Our new integrated care program is designed to further enhance our new, already highly successful, facility and clinical expansion that was completed in the spring of 2012, called Prairie SurgiCare, LLC, This initial facility and program enhancement was designed to offer unique surgical solutions for patients with spine and pain conditions. Now with the addition of our PowerCore for Life Program we will have completed another major component in our quest to offer truly integrated care within our IPU environment. It is because referring providers (i.e., family practice physicians and other specialty providers) continue to look for new solutions and unique care offerings for their patients that suffer with spine and pain medical conditions that we are building our new integrated care program. Based upon the input from referring medical providers we will initially offer the following integrated care services through our PowerCore for Life Program
