Primary Care
If you are a member of a Managed Care Insurance plan or HMO, and you are coming in for an office visit, a referral or prior authorization is required from your primary care physician. If we do not have the necessary authorization prior to your visit, your appointment will be rescheduled.
Sleep Disorders
Sleep disorders can manifest in many forms, and may be caused by a variety of neurological and physiological issues. Whether you are having problems falling asleep, staying asleep, or not feeling rested after sleep, a clinical sleep study may help to determine the cause. Many sleep disorders can be diagnosed through a sleep study, and the results can be used to diagnose an underlying problem and develop a course of effective treatment.

Any medical advice will need to be provided by the physician or mid-level during your appointment. Please go to the nearest emergency room if symptoms are persistent and you are unable to wait for your appointment.
