The Homestead is one of the long-term care facilities operated by Finger Lakes Health. Based in Penn Yan, N.Y., the center offers a variety of long-term care services through trained physicians and personnel specialized in various areas, such as pharmacy, physical therapy, dietary, social work and activities. It conducts the adult day health services program that provides health care and social interaction services for adults, and support and counseling for family and caregivers. The Homestead also offers the transitional care program that provides rehabilitative services to improve the functional independence of patients with hip and knee replacement, fractured bones, amputation and cardiovascular accidents. Its program also serves patients who have faced an extended medical hospitalization for pneumonia, congestive heart failure and other heart ailments.
