Eleanor's Bras and Breasts Forms has provided the highest quality of both service & selection to breast cancer survivors for nearly 30 yrs. and is accredited through The American Board for Certification in Orthotics and Prosthetics Inc. Our Fitters are both qualified and accredited & have over 45 yrs of experience in providing the most personalized attention and detailed fittings possible. We inventory a wide selection of mastectomy products, including a variety of custom-fit breast prosthesis, enhancer/lumpectomy shells, adhesive nipples, prosthesis covers, mastectomy bras, and post-surgery camisole. (We also offer mastectomy bathing suits, Lymphedema pumps, compression sleeves, stockings, gauntlets, & various other accessories;however, these items are not covered by most insurance plans.) We currently accepts assignments on Medicare, Humana, Universal, United Health Care, Care plus, Aetna, Tricare, Champva, various Medicare Advantage and certain BCBS claims. We Also submit non-assigned claims. (For reimbursement on behalf of the patient) to other (private) insurance carrier. We recommend patients familiarize themselves with their individual insurance plan. (Pre-authorizations, dollar amount and/or percentage covered, co-pay deductible etc). Prior to their fitting and offer. "In person" assistance in this area when requested. Our Office hrs are Mon-Fri. from 9am - 4pm. Appointments are required.
