For urgent (non-emergency matters) our existing patients can reach Dr Crouch or JaNena for guideance by phone 24/7/365 via our after-hours call coverage at 405-258-9955. Patients with true medical emergencies should always call 911. When you do need to visit an ER or have a hospital stay, we want to see you after you a released to help with your transition back to home. Our Hospital Follow-Up visits and ER Follow-Up visits give you the clarification and guidance you need to take care of yourself at home once you are released. We have found this time spent with patients to understand the hospital care at home orders and medication changes goes a long way to prevent re-admission. Please call us to schedule your follow-up visit and help us help you with your Transition of Care to stay healthy at home.   The clinic is a non-emergency, outpatient facility offering lab services, Digital X-Ray, and EKG. Dr. Crouch is Board Certified in Family Medicine through the American Osteopathic Board of Family Physicians. He is also certified as a Diplomate in Obesity Medicine by The American Board of Obesity Medicine, one of only 19 physicians in Oklahoma who hold this designation.

  We will review your medical history and immunizations and perform a simple physical exam to determine if you are able to safely participate in your chosen sport. We will fill out the forms needed by your school or coach to get you in the game! As an added benefit, your forms are securely uploaded into your chart for future reference.

He is Board Certified by the American Osteopathic Board of Family Physicians (AOBFP).
