I have seen the ball dropped by some of the staff but usually it was because they did not have adequate support from the upper management. Direct line staff were required to work and assure wholistic inclusion of a clients issues/concerns but sometimes did not have the tools to take care of the needs. This was somewhat frustrating and was the cause of some things not being accomplished.
