- Primary Care10. Medical Records. If you request that a copy of your medical records be sent directly to you or another third party, the practice will charge a per page fee, payable in advance. However, where your medical record is to be sent to a collaborating physician (primary care or specialist) to assist in your care, there is no charge. A schedule of Medical Records Request Fees is available upon request.
