  In order to expedite your first visit, please print and bring the completed forms below to your appointment.  If you choose to fill out the forms at our office, please arrive 10-15 minutes prior to your appointment time. Please bring the following documentation if applicable, all HMO/MCO/POS referrals and authorization from your Primary Care Physician and/or specialist. List of current medications and allergies.

Neurologists

• Autism
  After graduating she went on to specialize in the treatment of children with disabilities. For the past 19 years she has worked successfully in multiple settings. She has extensive  experience treating children with multiple diagnosis including Autism and Cerebral Palsy. Stella ‘s inspiration and passion for the treatment of children led her to want to help people of all ages. In 2009 she opened  Advance Therapy Works Rehabilitation Center with the goal of offering multi-discipline therapies to all ages. Stella has received specialized training for the treatment of the shoulder, hands and neurological disorders for adults. She has multiple certifications in the treatment of Autism. Since 2010 she has been the Therapy Director for the Marian Center a school for children with disabilities. Stella’s philosophy is to make sure that  her patients receive the personalized care that they deserve in order to help them reach their full potential.

  Graduated from Florida International University in 2011 and received her Doctorate in Physical Therapy. She is an active American Physical Therapy Association member and has worked in a variety of treatment settings including outpatient orthopedics with sports-related injuries, acute care, and pediatric rehabilitation.

Physical Therapists

• Sports Medicine
  We specialize in pre-surgical and post-surgical rehabilitation, chronic conditions, sports injuries as well as the development of a rehabilitative program for patients of all ages, so they can achieve their optimal level of function.
• Physical Therapy
  We deliver high-quality physical therapy in a supportive environment with experienced therapists that provide expert treatment of all musculoskeletal conditions.
• Occupational Therapy
  An occupational therapist certified by the National Board for Certification in Occupational Therapy. She was awarded her Bachelor in Science in Occupational Therapy from Florida International University in 2000.
• Manual Therapy
  Graduated from Miami Dade College-Medical Campus, 2001 with an Associates of Physical Therapy, Board Certified. Her specialty is in Orthopedics, Sports Injuries, and Geriatrics. Cathy works closely with each patient using therapeutic exercises and manual therapy approach to attain maximum rehabilitation potential.
