Legacy Home Health Care began operation in 2002 to provide geriatric care, rehabilitation and other services. As a home health provider, Legacy Home Health Care is committed to provide a broad range of quality, intermittent services and programs to patients of all ages. Legacy's health care team includes registered nurses; physical, speech and occupational therapists; medical social workers; LPNs, and certified homemakers/home health aides. Legacy Home Health Care has attracted the finest nurses, therapists and other professionals in the communities it serves. In addition to providing the highest quality of services available, continuity of care is also its top priority. On-call nurses are available by telephone 24-hours a day, 7 days a week. Primary care nurses assigned to patients make regular and supervisory visits as ordered. During supervisory visits, nurses evaluate the care given as well as review the overall family dynamics. Legacy Home Health provides professional services to Medicare, Medicaid and third party payers.
