Embassy of Park Avenue is a skilled nursing and rehabilitation center, located towards the end of Park Avenue Extension. We offer long-term care to many friends, family, and neighbors from the tri-county area. We also specialize in short-term rehabilitation- physical therapy, speech therapy, occupational therapy, and therapeutic modalities focused on getting patients home sooner and decreasing a return to the hospital. Over 200 employees, full-time, part-time, and PRN, provide quality care at the Embassy of Park Avenue.
