Health Care Attendant Program: This program is designed to work with individuals to keep them living comfortably and independently in their own homes, enhancing their quality of life without the stress of interrupted routines and changes in daily habits.Our staff provide these helpful and necessary services that reduce the difficult, time consuming and often stressful aspects of family caregiving:Bathing, Dressing, Meal Preparation, Shopping, Light Housekeeping and Medication Reminders.Health Care Attendant services are available each day of the week, on the weekends and during the early morning and late evening hours. Home Health Program: Home Health care can be the perfect match for many individuals as they recover from an illness or injury in the comfort of their own home. It helps them regain their independence and become as self-sufficient as possible.The Home Health team provide and help coordinate the care and/or therapy that the persons doctor orders. Along with the doctor, the Home Health team create a plan of care to reach and maintain the persons best physical, mental and social well being. Transitions: Transitions is a free community service available to anyone dealing with the challenges of a life-limiting illness. An important part of the Transitions program is the companionship, assurance and support that are so vital to those facing complex issues relating to their illness, treatment and outcomes. The Transitions staff provides case management services to identify client and family needs and offers resources to fulfill those needs through volunteers, referrals and links to community agencies. Hospice: Hospice is a specialized, compassionate concept of care that recognizes hope as a powerful force that sustains life throughout a serious illness. An early referral to Hospice is of great benefit to the client, caregivers and family members. Individuals and their family may discuss Hospice care at any time with their health care professional - doctor, nurse, hospital staff, etc. or meet with Homecare & Hospice staff at any time to discuss hospice. The Hospice team cares for clients in their homes, nursing homes, assisted living centers, hospitals or in the home-like environment of The Good Shepherd Hospice House. They help clients to be as comfortable as possible through expert pain and symptom management, supporting them and their families, providing respite to the caregivers and other practical assistance.
