The vaulted ceiling and skylights of the one-story building give our facility a bright, open feeling. Three enclosed courtyards welcome you with flower gardens, patio furniture and park benches. There are four tastefully furnished lounge areas ideal for relaxing, socializing or reading a book. And, within these elegant surroundings, you will find short-term as well as long-term health care and rehabilitation services that are second to none. Summerfield Health and Rehabilitation Center offers both private and semi-private rooms, all Medicare approved and each with a window to a lovely view. Our interdisciplinary team for transitional care brings the skills of a variety of specialists to work toward your recovery. You get the benefit of their knowledge in developing and implementing a personalized care plan designed to speed your recovery and keep everyone involved during the transition process. Weekly updates are provided by the social services director so that you will always be informed.
