The main Los Olivos office is 15151 National Avenue and the Los Olivos South office is located next door at 15195 National Avenue, Suite 207, above the Los Gatos Surgical Center next to the main building. Los Olivos Mountain View is located at 2500 Hospital Drive, Suite 4A in Mountain View, CA 94040. Obstetric and Gynecology appointments are available at all three sites.

- UltrasoundPlease schedule an appointment at Los Olivos by calling or emailing the receptionist of the physician that you would like to see. Most physicians will schedule you for a pregnancy confirmation visit that includes an ultrasound for your first visit. You will receive a copy of the Los Olivos Pregnancy Book at your first obstetric appointment. Please complete the Genetics Questionnair e and bring it to your first appointment. If you have not been to Los Olivos in the past, please also complete the New Patient Questionnaire.
