"Providing assistance for seniors and disabled adults with Information and Referral, Care Management, Medical and Insurance Benefits, Medi-Cal, Veterans Benefits and multiple resources on a short-term and long-term basis. This service is considered to be private geriatric and disabled care management. I strive to assist clients and their families with maintaining and enhancing their quality of life under the best possible circumstances. I will visit the client in their home or in a facility in order to provide ongoing monitoring assistance, initial assessments and coordination of services. Meetings can be arranged according to your needs.
