  Many policyowners want to change the name of the beneficiary or include more beneficiaries on their life insurance policy. The process is simple. You may contact your financial professional for assistance. You may also call our toll-free Customer Service telephone number at 1.800.233.7979 and request a Beneficiary Change Request form. We will mail it to you along with instructions on how to fill it out. (A copy of this form may also be printed from the Policy Forms section of this site.) Please reference the Legal Matters brochure to help you determine appropriate beneficiary changes.

  We cannot always predict the future, but long-term care is a way to help secure peace of mind. We provide employees the opportunity to purchase long-term care coverage through DMBA or one of our strategic partners at a reduced rate payable through payroll deduction.

• Annuities
  An understanding of the Goodman Rule is helpful in ownership and beneficiary changes. The result of the Goodman Rule is that for life insurance and annuities, the owner must also be either the insured (annuitant) or the beneficiary. If the owner, insured, and beneficiary are three different persons, then the death of the insured results in the owner making a taxable gift to the beneficiary.
