General Info

We provide quality community based heath care services to individuals who suffer from chronic mental and physical impairment including Alzheimer's Disease and other forms of Dementia, down's Syndrome, Cerebral Palsy, Mental Retardation, Brain Injury, Stroke Survivor, Schizophrenia, Diabetes, Chronic Hypertension, and/or those who are in need of rehabilitation services to maintain or improve their level of functioning.
