Our team consists of: - Medical Director who oversees plan of care - Physicians who follow care of patient - Registered Nurses who make schedule visits - Registered Nurses on call 24/7 - Home Health Aides, Social Workers, Spiritual Givers, Bereavement Counselors, Physical Therapists , Dieticians and Trained Volunteers that attend to patient needs and provide caregiver support
