AboutCentral Oklahoma Family Medical Center (COFMC) began in the early 60 as community leaders recognized the need for closer health services. Dedicated in 1966, the first health center included two physicians and one dentist.Two decades later, after the two physicians had left, community members and the Central Oklahoma Economic Development Agency collaborated to write a grant for a new community health center. A board of Directors was established and by 1985, Konawa housed the first rural community health center (CHC) in the state of Oklahoma.Today, a Board of Directors still exists made up of community members. COFMC now has two locations, and includes medical, behavioral health, dental, pharmacy, lab/x-ray.MissionThe mission of COFMC is to provide patient centered, community-based health care through comprehensive and coordinated care without regard to finance, culture, or lifestyle by providing the education and support to promote participation in their own health care decisions.VisionThe vision of COFMC is to provide the finest primary and preventive healthcare network to patients in our service area. By developing a strong organizational structure through our board of directors and staff, COFMC is able to provide a foundation for the health center, which is the catalyst for all organizational decisions.
