- Emergency CareSevere chronic pain, nausea, or seizures may allow you to obtain a medical marijuana card if these conditions interfere with your daily activities - working, sleep, leisure activities, family interaction. We will need medical records: typically we ask for the chart notes from the 5 most recent doctor visits pertaining to your qualifying condition. These records would reflect your name, a doctor's name, and notes about the condition. They can be from different doctors, and may include specialists or emergency room visits
- GlaucomaA diagnosis from your doctor is all we need if you suffer from cancer, HIV/AIDS, glaucoma, or ALS. For everything else, we need records from THREE visits WITHIN the last three years about your qualifying condition.
- EndometriosisJen D., 30, of Caledonia broke her neck in a car accident in 2006. During her recovery - which included surgery to fuse bones in her neck - she was unable to tolerate prescription painkillers. She knew from past experience that marijuana would help her pain, having smoked it to ease painful endometriosis in the past. She was happy to become certified as a medical marijuana user in December 2010. "Its hard to do anything when you're in constant pain. Marijuana has helped me tremendously, and it works without turning me into a zombie. I'm so happy that I'm legal now."
- Ovarian CancerDave L., 58, of Constantine lost his wife to ovarian cancer in December 2010. As Kay's treatments proved more debilitating over time, the couple decided to try marijuana, which eased Kay's suffering in her last months of life. "My wife really liked Dr. Crocker. He talked to her like a friend, not like a doctor. He said he wished we'd come to him sooner. I wish we had too, because I saw how much better she felt during her last two months using cannabis."

"Marijuana had a lot to do with me surviving my cancer."
