Community Health Center is a nonprofit organization that offers a wide variety of home health care services to the residents of Johnstown, N.Y. It offers various short- and long-term care services for its residents, including skilled nursing assistance, respiratory therapy, nutrition counseling, occupational, speech and physical therapy, wound, ostomy and continence assistance and intravenous therapy. The center also provides certified home health aides, medical social workers, community mental health nurses, personal care aid and housekeepers. Its special programs include diabetes management, developmental services for preschool children, disease management, nutrition and wellness program, and heart and home adult day center. The organization also educates patients about their condition and trains them for wound care and managing diabetes.
