Located in Jamestown, N.Y., Lutheran Home and Rehabilitation Center is a Medicare- and Medicaid-certified 229-plus-bed skilled nursing facility that provides 24-hour medical assistance services for more than 250 residents. It prepares individualized care plans, as well as offers physical and occupational therapies. The center employs nurses, social workers and dieticians. Additional support is provided through pastoral care, volunteers, medical records and environmental services. Lutheran Home and Rehabilitation Center provides advanced clinical services in the areas of intravenous medication and feeding, respiratory and dementia care, peritoneal dialysis and wound management. The center is certified and licensed by the New York State Department of Health. It is a division of Lutheran Social Services, which offers volunteer support and housing assistance programs.
