Country Gables **Based on observations, interviews, and record reviews, Robin Gregorius, Registered Nurse (RN), Governing Authority, Administrator, failed to ensure the facility was operated according to the SBOH's rules and failed to properly perform her duties as administrator to ensure the facility was managed responsibly. Ms. Gregorius's inability or unwillingness to responsibly manage the facility resulted in multiple incidences of residents' rights violations and in an environment that was not safe or secure for residents. Numerous deficient practices were observed during the survey including physician's orders not being followed; care provided to residents without the direction and supervision of the physician.
