Patients must have a current Veterinary Health Form and/or an immunization record on file or other proof of current vaccination.
- Carpal Tunnel SyndromeAn Upper Quadrant/Carpal Tunnel Test is used to determine the capability of the prospective employee in relationship to repetitive job function, as opposed to testing physical strength. This test may be done in correlation with a Post-offer, Pre-Employment Test, the Fit-for Duty (for injured workers), or independently as its own stand alone test.

Currently, we require documentation that is no more than a year old from a certified mental health professional indicating need for an emotional support animal, plus certification of the animal’s health including rabies shot verification.
