Founded in 1996, Hart Community Homes is a residential treatment facility that serves homeless and at-risk youths between the ages of 13 and 18. It provides advocacy, employment, housing and mental health care services for alumni and youths in foster care systems. Hart Community Homes offers programs related to life skills development and workforce training, education, support and self-awareness. It provides a Department of Housing and Urban Development- approved home resale program that offers housing options through theownership and management of rental units for low-and-medium income buyers. Hart Community Homes conducts workshops on topics related to planning, case management, and problem solving and decision making skills. It is located in Anaheim, Calif.
