Conducting comprehensive physical, safety, and psychosocial assessments of older adults Developing and implementing customized plans of care based on assessmentInterviewing, selecting and providing oversight of in-home staff to ensure clients needs are well me Anticipating client/caregiver issues to proactively address and maintain quality care Facilitate medical treatment plan and pharmacy oversight Communicating/documenting all activities with appropriate parties Have extensive knowledge about the costs, quality, and availability of resources in their communities Travel locally to attend patient medical appointments Plan, coordinate, and implement all patient care, identifying and addressing all concerns Act as liaison between the geriatric client, and all parties involved in case (i.e. attorney, family, and physician, etc.) Complete comprehensive written monthly reports in a timely manner Participate in nursing staff educational teleconferences and meetings
