- Primary CareScheduling an appointment with our doctor is fast and easy; simply call our office and our helpful front office staff will coordinate an appointment. We see emergent patients on the same day and all other patients will be seen within 48 hours. In order to schedule an appointment we’ll need to gather personal information about you such as your full name, date of birth, address, health insurance ID number, etc. We take your privacy seriously and have identity security measures in place to protect your personal health information. Many appointments are scheduled as referrals from your primary care physician and we’re happy to coordinate care with your regular doctor. We’ll provide your doctor with feedback on your visit within 24 hours.
- Emergency CareEmergencies do occur and we ask that you not hesitate to call with urgent, urologic problems. After hours, our phone is covered by our answering service. Dr. Albert is always "On Call" for emergency care (24 hours/day, 365 days/year). The service will relay urgent questions regarding your urologic care to Dr. Albert.

- Mental HealthRight to Inspect and Copy. You have the right to inspect and copy your health information, with limited exceptions. To access your medical information, you must submit a written request detailing what information you want access to, whether you want to inspect it or get a copy of it, and if you want a copy, your preferred form and format. We will provide copies in your requested form and format if it is readily producible, or we will provide you with an alternative format you find acceptable, or if we can’t agree and we maintain the record in an electronic format, your choice of a readable electronic or hardcopy format. We will also send a copy to another person you designate in writing. We will charge a reasonable fee which covers our costs for labor, supplies, postage, and if requested and agreed to in advance, the cost of preparing an explanation or summary, as allowed by federal and California law. We may deny your request under limited circumstances. If we deny your request to access your child's records or the records of an incapacitated adult you are representing because we believe allowing access would be reasonably likely to cause substantial harm to the patient, you will have a right to appeal our decision. If we deny your request to access your psychotherapy notes, you will have the right to have them transferred to another mental health professional.
