Founded in 1983, Physicians Health Plan of Northern Indiana is a physician-sponsored nonprofit health maintenance organization. It focuses on preserving the community's access to quality and provides affordable health care service. The organization is governed by area doctors and business representatives. Physicians Health Plan of Northern Indiana provides its members open access to more than 2,000 network providers, including doctors, dentists, hospitals and medical facilities. It provides coverage for tests and screenings, various discounts and educational literature. The organization also provides coverage for mammogram, Pap smear, immunization, well baby care, and colorectal and prostate screening.
