  My office is located behind the Blanchard Valley Hospital at 1818 Chapel Drive Ste C.  When traveling on Main Street, turn West on Pearl Street.  The street curves to the left, and becomes Chapel Drive.  You should see hospital staff parking lot number 5.  We are located in the next parking lot.  Enter by going to the drive just south of the entrance to parking lot number 5 and we are in suite C.  Our office is shared with the Gastroenterology Associates of NW Ohio
