Golden Years Nursing Home has proudly served the Falcon community since 1965, providing a complete continuum of care that includes short-term rehabilitation, long-term living, outpatient therapy, skilled nursing care, hospice care, palliative care and respite care. Golden Years is located in a serene country setting that is easily accessible from I-95. When you arrive at Golden Years, you are greeted by a clinical team who develops a person-centered care plan designed to help you regain skills and decrease the chance for readmission back to the hospital. You also have access to medical services that address secondary conditions for a complete treatment plan.
