Laurel Creek Health Center, a part of Paradise Valley Estates, provides skilled nursing and short term care programs to patients. The center provides assisted living services to patients recovering from illnesses, injuries, strokes and diseases. The services offered by the center include respite care, medication management and monitoring, nutrition and diet counseling, and pain management. It also provides rehabilitation services that include speech and language, physical and occupational therapies. It also operates special living units for residents with cognitive impairments and Alzheimer s disease. Laurel Creek Health Center is located in Fairfield, Calif., and is approved by the California Association of Homes and Services for the Aging.
