The HMO patient is responsible to obtain the required written referral. As the provider, we are required to follow these policies to seek reimbursement from your insurance company for any office visit.

- Internal MedicineEjikeme Obasi, MB.BS, FACP – Dr. Obasi received his undergraduate and medical degrees from the University of Nigeria. After completing a residency in Internal Medicine in Nigeria and at Cook County Hospital Chicago, he went on to Nephrology fellowship training at Rush University, also in Chicago. Dr. Obasi is board certified in both Internal Medicine and Nephrology. He is currently an Assistant Clinical Professor of Medicine with the University of Illinois and acts as Medical Director for several dialysis units, including the Merrionette Park Dialysis Facility and the acute Dialysis program at Little Company of Mary Hospital. He is the current President of Southwest Nephrology Associates. His special interests include Home Dialysis therapies, Diabetes and Chronic Kidney Disease.

- Kidney StonesAmishi Patel, M.D. – Born in Toronto, Canada, Dr. Patel relocated to the United States in 1998 and received her undergraduate degree from the University of Illinois at Chicago where she graduated with honors and went on to complete her medical degree at Southern Illinois University School of Medicine. She completed her residency and fellowship at the University of Illinois at Chicago. She is board certified in both Internal Medicine and Nephrology. Her special interests include kidney stones and she offers a specialized stone clinic. This clinic provides a comprehensive treatment plan individualized to each patient by working closely with urologists with the goal of prevention of further kidney stones. Other interests include chronic kidney disease and dialysis therapies.

- Occupational TherapyRetired from Southwest Nephrology Associates in January 2017 and is currently the Chief Medical Officer at Advocate Christ Hospital. – Raised and educated in Milwaukee, Dr Guglielmi received her Bachelor’s degree at the University of Wisconsin – Milwaukee. After working in occupational therapy for three years, she went on to graduate from the Medical College of Wisconsin where she was a member of Alpha Omega Alpha. She completed her Internal Medicine Residency, Chief Residency and Nephrology fellowship at the Medical College of Wisconsin after which she joined Southwest Nephrology. She is board certified in both Internal Medicine and Nephrology. She is a Clinical Associate Professor of Medicine, Rosalind Franklin University. Dr. Guglielmi serves as medical director for the outpatient DSI dialysis facility on 79th and Cicero. She is also the medical director of the inpatient hemodialysis unit and the continuous renal replacement program at Advocate Christ Medical Center. She is currently the President of the Medical Staff at Advocate Christ Medical Center where she is also a member of the cardiac transplant/ventricular assist device team. Her areas of special interest include cardio-renal syndrome, hypertension, renal disease in pregnancy, and continuous renal replacement therapy.
