We provide geriatric care management services to assist families in providing the best possible care for elder loved ones, whether in home, an assisted living facility, or nursing facility. We act as liaison between our clients, their families, medical professionals, home care agencies, facility staff, etc. Everything begins with an assessment of physical, emotional, and social needs, development of a plan of care and recommendations for in-home assistance, home equipment, elder law/legal services, financial/bill-paying services, facility care, etc. as needed.
